Fuel injectors sticking or stuck
Fuel injectors are designed to snap shut and immediately cut off fuel supply. If they are sticking or stuck, fuel can continue to enter the combustion chamber when it's not supposed to.

Symptoms
Exhaust Blows Black Smoke
If the fuel injectors are not fully closing, then more fuel will be admitted to the combustion chamber. This will alter the ratio of fuel and air, causing black smoke to exit the exhaust as the fuel is burned.
See excess fuel in the system.
